1. Make a claim with your insurance company

2. Book an appointment with your family doctor

3. Fill in an OCF-1 & send to your insurance company

4. Provide OMWC with the following information:

- Date of accident

-  Insurance company’s name

-  Insurance policy number

-  Insurance claim number

-  Insurance adjuster’s first & last name

-  Insurance adjuster’s phone number

-  Insurance adjuster’s fax number

-  Doctor’s name

-  Doctor’s phone number

-  Doctor’s fax number

5. OMWC will create a treatment plan, which you then have to sign

6. After you sign the plan, OMWC will fax it to your doctor for approval

7. After OMWC receives approval from your doctor, we will fax it to your insurance adjuster for approval

Please note:

- If you have coverage for treatments through other insurance, you will be required to exhaust that coverage before receiving treatments under your MVA treatment plan

- You will be required to pay for treatment until OMWC receives approval for the plan
